Welcome aboard! Please fill out the following paperwork items by the listed due date.
NOTE: We ask that you upload any and all documents containing sensitive information directly to our secure online storage via the links provided below. Please do not email sensitive documents. Contact info@compasspath.org with questions.
Some of the forms listed below will need to be 1) filled out with a PDF editor, OR 2) printed, filled out, scanned/copied, and then uploaded. You can also submit camera pictures of printed documents, but please ensure that all edges of the documents are visible, that the writing is legible, and that the writing is not blurred or obscured by a lighting glare.
*If you already uploaded a profile photo when you registered, then you do not need to upload another one.
Please submit the following items by October 1st
*If you do not have a passport, you will need to apply for one by October 1st and upload your application receipt to this folder in lieu of the document itself.
The following forms are required only for minor participants who will not be 18+ by the start of the trip.
*This can be a birth certificate, adoption certificate, custody decree, etc. Please save image as PDF, PNG or JPG titled “First Last Child Proof” then upload.
